As for the "holes" ya, Vasto Lordes will have them, because they're the "slots" in which the chains which bound them to life originally fitted in, all souls, hollows and so on have them.

And though it's not a main topic, on the subject of the Grand Fisher.
I believe that he is a menos adjuchas due to his unique abilities, the lowest form of hollow we've ever really seen to have those types are huge hollows, which if you look back are pretty easily handled by the shinigami. Now that on first view suggests he's a huge hollow, but as is explained he's been around for 50 years and causing the shinigami a lot of trouble, so if he's so tricky then the chances are he's a menos.

And for the arrancar.
I believe skin colour and so on are dismissed as not counting as human characteristics in something like this, because chances are the colours are just used artistically to reinforce people like Ulquiorra's personality characteristics, so in actuality he looks nearly completely human, just a little moroes.
I can't help but think that none of the Vasto Lordes have been discovered yet, simply because the power they'd have just doesn't seem proportional to the Espada.

Many normal hollow have unique abilities... Rewatch some of the earlier episodes, take for example;
Shrieker - the hollow that could create bombs attach them to you, and explode them with the pitch of his tounge.
Or the hollow that Orihime first fought, that could take control of a certain body part.

Grand was most likely just a Gillian, and made into an incomplete arrancar, ie. his size.

But you still can't get me to believe Ulquiorra looks human. Take a moment and compare him with our known top 3 espada. Them looking more human isnt strange, since the more intelligence they have the more human they look. Or it could be due to the fact the the top three are Vastros, and this is supported by the 3 ex-espada.

It's impossible for him to have been a Gillian, he didn't have the appearance of one.

And to think of it, Shrieker was a large fella, and it was told that he had been around for a long time, toying with people and killing them through the child's soul. So again, he was large, he was intelligent enough to speak (unlike the usual average hollows) and he had a unique ability.
Working by association with other Hollows who have a known type, I'd classify him as a huge Hollow at least.
