Well, the furthest an arrancar has taken it so far, has been the Segunda Etapa that Ulquiorra showed us, and we might assume that it is the final stage of a hollow's release.

My own hypothesis:
What Ichigo did unintentionally against Ulquiorra was something similar to a stage two resurección (highly debatable), and we know for a fact that Ichigo became a hollow AND a shinigami in Urahara's pit, which means that since he looked completely human in his spirit form, that maybe he passively locked all of his hollow powers into that same sword, similar to how a hollow does in order to gain/use shinigami techniques.

If this somehow could be correct or whatev, then that means that there is the possibility for a vizard to get both Bankai AND a Segunda Etapa, and the same for arrancars.
Why I think we haven't seen anyone with these traits (yet), is because they'd have to be extremely skilled at spiritual energy manipulation from both sides of their Hakusui, or Soul Sleep (Just scroll down), and we really haven't seen the full powers of the vizard yet.

^^wow,i freaking love that theory.It woul explain why he doesnt truly have a bankai(although he keeps the sword in his second hollow form).neither does he have a true ressurection(or at least a themed hollow release...he is straddling the line but approaching the pinnacle of both sides all the same.
ichigos mask as well as as his zanpakto are both ever-present when he is in spirit form(though the same is true of the vaiards & arrancar).

finally a possible purpose for Tensa-Zangetsu

I doubt Isshin would have but Urahara may have. Could have been unintentional. Or he could have been a Vizard from the start.
