I know who two of them are, but the guy at the top, is beyond me.

The one removing Grand Fisher's mask is Iceringer, the arrancar Ishida killed when they first came to HM. Then we have Grand fisher, and the unknown guy with his worm at the top. Who seems to give the orders out.

In my opinion, a said crew of naturally born arrancar who called themselves Espada or maybe even something else existed pre-Aizen. Aizen just finished the job with the hogyoku if they agreed to follow him. It is said several times the "ORIGINAL" espada, met a tragic demise.
They were either killed off for not following Aizen, or as seen in Nnoitra's flash backs killed by the new espada.

As seen in the pages, a group of hollows band together, removing their masks naturally. A group of arrancar pre-Aizen, and pre-hogyoku arrivial in HM.

And the way SS has been run so far they would not call the royal gaurd out. They start with the underlings and the stronger people only are dispanded when it is deamed nessisary. They would keep their last line of defense in reserve till absolutely needed.

exactly, thats why captains were dismissed almost imediately after ichigo invaded SS.

The Royal guard is above the central 46, who command everything soul society does. Yamamoto has no authority to call apon the royal guard, same goes for central.

Unless the king directly feels unsafe himself, which is doubtful considering hes in his own dimension, he wont send the royal guard out.

huh? i wuz agree wit wht dborza (sp) said lol i think aizen is on the lvl of royal guards, he cud fight on par with them, 1 at a time but dats it the reason i say they wudnt call royal guards i bcuz yama is confident is SS and yama is very powerful himself right now as ive said before yama and shinji r the only 2 i believe can take on aizen without gettind owned

true but yama is in charge of SS now so if he realli needed the help i guess he cud make a plea of some sort

I believe Kensei, Love, and Rose are just as powerful as Shinji.

y cuz they were captains also, remember even as captains there will be a diff in power with shinji being the leader i think it's kinda obvious he is the strongest but who knws anything is possible

im sure if yama said "hey, theres iminant threat to the king" they would send someone.
