Hollows are former (deceased souls) that lose their hearts to despair or remain in the real world for too long. Any spirit that is not guided to Soul Society by a Soul Reaper may eventually descend into a hollow.

When their soul chains vanish, their bodys detorate, and reform, then the mask is spit out.
His hollow, was created by his soul, so it was his hollow, didn't come from anywhere.

Im guessing if your soul is strong enough to resist transformation, you have shinigami powers, and can choose that route. But still have that hollow you could of been, inside your soul.

but ichigo wasnt dead they hollows are formed by dead sould that have not been takin to soul society he had to come from some where such as his mom
