Hollows come from souls, and reside in HM they don't come from it. We don't know how Ichigo has an inner hollow in the first place, but we know why the Vizard have em'.

hm but ichigos hollow had to be dead on earth at somepoint..................his mom holy shit that would be the perfect thing ok she dies tryin to save ichigo but left her family behind i mean that sound like a theroy

Maybe if the soul is strong enough to resist the hollow transformation, the mask comes out first, and the body doesnt detorate like it would if you couldn't.

So basically he died, and was able to harness his Shinigami powers, but having the hollow transformation already in progress, it couldn't just disappear. The hollow was just hiding deep inside his soul since the shinigami powers won that battle.

a hollow is created when someone dies and has something the didnot do while on earth then when there soul chain vanishs they go to hm and are eaten by hollows and become one themseleves hance the name hollow
